If your Oklahoma certification has lapsed

The cheaper route — while it lasts

OAC 310:677-5-4(a): after 24 consecutive months with no compensated nursing or health-related services, you must complete a new training program and competency evaluation, OR a new competency evaluation only — the choice is yours. The Department's Retest Application (OAC 310:677-1-3(g)) covers aides who missed the 8-hour rule in the 24 months before expiration, up to 24 months after expiration, or whose certification has been expired more than 2 years but less than 3 years. Fail the approved retest and you must complete new training before any further retest.

Past that line

After 48 consecutive months without applying for renewal, your certification is moved to the archive automatically (OAC 310:677-5-4(d)): reinstatement then requires a new nurse aide training program and the competency examination. One exception: documented continuous employment past the 48 months in a hospital, the DHS Advantage Program, or another non-certification setting may be excepted under 5-4(e).

Oklahoma certifies several aide types (long-term care, home health, ICF/IID, residential care, adult day care) with different renewal paperwork. The free online renewal described here is for Long Term Care Aides; other types file a paper renewal with a $10 fee per certification type. Testing is multi-vendor: OSDH accepts Headmaster (most widely used, via TMU), Prometric, and the Health Certification Project, which is why no single exam fee is listed.

Transferring INTO Oklahoma

Application fee: $0 · typical processing: About 30 days processing per secondary summaries of OSDH practice.. Must hold active certification in good standing in another state with no substantiated findings; applicant must list every state where certification was ever held, disclose convictions with court documents, and file an Affidavit of Lawful Presence (ODH Form 301). Use Reciprocity Application ODH Form 735. Long Term Care Aide reciprocity is free; Home Health Aide reciprocity costs $15. LTC applications can be emailed to nar@health.ok.gov.

The table below is computed for an active aide in good standing, trained to the origin state's standard hours. Your own hours or status can change your row — run the transfer checker with your real facts.
