New York CNA certification: stay active, lapse, and transfer rules

Your employer recertifies you. To be recertified you must have worked at least 7 hours for pay as a nurse aide in a New York nursing home or other NYSDOH-approved nurse aide employer within the previous 24-month period, under the supervision of a licensed nurse. The employer submits the recertification and pays the $40 fee.

Is my certification still active?

If you have never worked for pay since certifying, use your certification date.

If your New York certification has lapsed

The cheaper route — while it lasts

If you did not meet the work rule, you must retest (both Clinical Skills and Written/Oral exams). Retest without new training (Route 6) is available if you were certified through a NY-approved program on or after July 1, 1989. If your certification came through deeming, waiver or reciprocity, or your training was before July 1, 1989, you must complete a new approved training program before reapplying (Route 7).

Past that line

If you did not meet the work rule, you must retest (both Clinical Skills and Written/Oral exams). Retest without new training (Route 6) is available if you were certified through a NY-approved program on or after July 1, 1989. If your certification came through deeming, waiver or reciprocity, or your training was before July 1, 1989, you must complete a new approved training program before reapplying (Route 7).

Do not file your own renewal and wait: in New York the employer drives recertification, so a job change near expiration is the classic way certifications lapse. Exam fee splits for first-time testers are bundled ($115 written route / $135 oral route); retests are priced separately (skills $68, written $57, oral $67).

Registry findings in New York

New York has a published process to petition for removal of a single, neglect-only registry finding after at least 1 year. Abuse and misappropriation findings cannot be removed. Details — free, and with no strings — on the registry findings page.

Transferring INTO New York

Application fee: $50 · typical processing: About 30 days average processing (NY Business Express). Prometric Route 2 (Reciprocity/CNA from another state): active out-of-state certification in good standing and a $50 application processing fee. Submit a copy of your current out-of-state certificate. If your home-state certificate/registry does not show an expiration date, you must instead prove at least 7 hours of paid nurse aide work in the past 24 months with a signed letter on facility letterhead from your supervisor stating you performed nurse aide duties. Work-history requirement: At least 7 hours of paid nurse aide work in the past 24 months, required as proof when the home-state certificate has no expiration date. Prior verified research for this dataset notes the fuller paper trail behind Route 2: notarized training records and a verification form completed/mailed by the home state registry may be requested - budget time for the home state to respond, not just for Prometric.

The table below is computed for an active aide in good standing, trained to the origin state's standard hours. Your own hours or status can change your row — run the transfer checker with your real facts.
